The Black Coalition for AIDS Prevention (Black CAP) is a volunteer-driven, nonprofit, community-based organization. The mission of the agency is to reduce the spread of HIV infection within Toronto’s Black communities while enhancing the quality of life of those living with or affected by HIV/AIDS. We work in partnership with organizations and individuals who support in principle and practice our mission, philosophy, and activities.
Position Summary:
Black CAP’s Youth Outreach Program is a multi-agency outreach initiative for at-risk Black HIV-positive, queer, questioning, and Trans youth aged 12-25. The Program will provide individualized and group supports to youth and their families/caregivers to address the impact of violence, HIV stigma, transphobia, homophobia, anti-Black racism, and other determinants of health such as homelessness, substance use, mental health, etc.
The Program will include counselling and support services approaches that incorporate Afro-centric, rites of passage, resiliency, and stages of change frameworks. The Program will also actively engage HIV-positive and LGBTQ Black youth using a collaborative, person-centred approach grounded in trauma-informed practice.
The Program is funded through the Ontario Black Youth Action Plan’s Enhanced Youth Outreach Worker (EYOW) program. EYOW services build resiliency from a culturally specific model and achieve outcomes related to reducing the impact of homophobia/transphobia and HIV stigma among Black youth.
EYOWs work with Black youth to develop and enhance their identity, culture, and sense of belongingness to their community as a protective factor in their movement through the Stages of Change. EYOWs act as mentors, promote broader opportunities and supports for youth within communities, and advocate for the improvement of access to youth services. More specifically, EYOWs:
- Help youth and their families to identify and articulate their future goals and needs.
- Support youth through an evidence-based process to make positive changes in their lives.
- Raise awareness about, and access to, local prevention and intervention resources through information-sharing and referrals that respond to individual needs and risk factors and reinforce healthy choices and strengths.
- Foster communication and linkages among community agencies/organizations to improve access and reduce barriers to services and supports.
Please note this position requires a Bachelor’s of Social Work or Counselling Psychology or equivalent and certification/membership in a registered professional association for practitioners in clinical therapy. Key responsibilities of this position include:
- Provide short-term, counselling and support services to Black youth in accordance with the Stages of Change intervention model adapted for the YOW Program.
- Develop and oversee a short-term case management plan for Black youth clients.
- Collaborate with the YOW network to identify Black youth who may require additional, more intensive services, and work with the YOWs to align support services for individual Black youth.
- Support parents and family members to help youth address their needs by providing short-term counselling, if needed, and referring parents and family members to other supports and services available in the community.
- Maintain collaborative working relationships with other professionals within and outside of the organization.
- Enter and maintain accurate and complete service data in the Outreach Record database in a timely manner.
- Participate in training opportunities through the Outreach Record Learning Portal.
- Maintain professional and evidence-based standards of practice in accordance with the Stages of Change intervention model, a positive youth development framework and relevant legislation (e.g., Child and Family Services Act, Youth Criminal Justice Act).
- Carry out other duties as assigned as part of the Youth Outreach Worker position.
- Conduct appropriate follow-up and complete monthly/annual reporting.
- Attending staff meetings and training.
- Evaluate program-related activities and complete monitoring and quality reports in order to ensure high quality programming.
- Collaborate with members of Black CAP’s support services team.
- Other duties as assigned.
We are seeking an individual with the following qualifications:
- Bachelor of Social Work or Counselling Psychology or equivalent and certification/membership in a registered professional association for practitioners in clinical therapy.
- A vulnerable sector check completed by local police service.
- Knowledge of HIV/AIDS, STIs, SBBI, and sexual health issues and the impact of HIV/AIDS in Toronto’s Black, African, or Caribbean communities with excellent skills and knowledge of community resources and support available.
- Demonstrated ability delivering intensive, short-term counselling and support services to Black HIV-positive and/or LGBTQ youth in crisis, helping them to address their specific challenges, and referring and supporting them to access required services.
- Demonstrated ability to support family members of high-risk Black youth and refer them to required services.
- Skills in managing the case plans of multiple, high-risk Black youth clients and their families.
- Experience researching effective strategies and models for peer-based programming for individuals and groups to inform program delivery.
- Experience providing system navigation supports and identifying appropriate referrals to complementary programming based on determinants of health framework.
- Strong written and oral communication skills.
- Self-motivated and able to work effectively under pressure while maintaining a positive and friendly attitude as you deal with multiple personalities, projects, and deadlines.
- Knowledge of MS Office (Microsoft Word, Excel, and PowerPoint), internet, and email are essential.
- Willingness to work flexible hours including weekdays, evenings, and weekends.

We provide support, education, and advocacy to individuals and communities affected by HIV, including the provision of comprehensive and culturally appropriate sexual health services.
Position Purpose:
We are seeking a Harm Reduction Outreach Coordinator responsible for managing Black CAP’s Harm Reduction (HR) Program. The individual must have experience working in, and preferably managing, a harm reduction program and effectively overseeing a team in a harm reduction setting. Black CAP’s HR Program conducts prevention and education outreach to a range of clients including youth, adults, clients living with HIV/AIDS, and LGBTQ people in areas of Toronto where there are geographic and social concentrations of African, Caribbean, and Black (ACB) people. The Coordinator will play a role in strengthening Black CAP’s response to substance-using communities and increasing levels of awareness of drug prevention and harm reduction for our clients, staff, and community partners. The HR Coordinator will support the part-time Peer Educator who will assist in HR outreach activities and will help to supervise placement students completing their practical experience in the agency. The Coordinator also works collaboratively with other members of Black CAP’s prevention/outreach team and other Black CAP staff and volunteers to achieve the objectives of the program/mission of Black CAP.

Key responsibilities of this position include:
- Coordinate weekly harm reduction outreach sessions in northern Etobicoke, Malvern, and southern Scarborough.
- Manage and coordinate the delivery of workshops to adult and youth participants in areas related to harm reduction, substance use, drug prevention, and the links between substance use and risk of HIV.
- Conduct outreach and facilitate partnerships with organizations serving Toronto’s African, Caribbean, and Black (ACB) substance-using communities and relevant organizations such as Toronto Public Health, the Harm Reduction Task Force, etc.
- Supervise a Peer Educator responsible for harm reduction outreach in ACB Trans communities.
- Train, recruit, and support three Peer Volunteers to conduct outreach.
- Co-coordinate a monthly after-hours drop-in session for the ACB substance-using population.

- Attend monthly staff meetings and trainings.

We are seeking an individual with the following qualifications:
- Firm understanding of Harm Reduction models/frameworks and knowledge of substance use in diverse ACB communities.
- Knowledge of HIV/AIDS, STIs, and sexual health issues and the impact of HIV/AIDS in Toronto’s ACB communities with excellent skills and knowledge of community resources and supports.
- Comfort conducting outreach, education, and the exchange of harm reduction supplies in a range of neighborhoods across the city. Ability to lead and motivate teams of volunteers and students in conducting outreach and education.
- Experience in building awareness of harm reduction, HIV, and STIs in Toronto’s ACB communities.
- Experience delivering harm reduction programming for women and sexual minority communities.
- Knowledge of, and relationships within, Toronto’s harm reduction service delivery system.

Position Overview: Black CAP is seeking an experienced and dedicated Program Coordinator to join our Programs and Services team. The Program Coordinator is responsible for developing HIV strategies, strengthening prevention efforts, and promoting HIV treatment adherence. This role will involve working on two Public Health Agency of Canada (PHAC) funded projects, focusing on addressing Anti-Black Racism (ABR) as a root cause and implementing an integrated national HIV prevention strategy for ACB (African, Caribbean, and Black) people in Canada.
Responsibilities:
Project 1: Anti-Black Racism as Root Cause: ACB Knowledge in the Fight Against HIV-AIDS
- Conduct desktop research on approaches to addressing ABR and community-led monitoring tools.
- Engage in in-person consultations with HIV Prevention Hubs.
- Facilitate online consultations and feedback processes with service providers and users.
- Develop an ABR index for HIV prevention.
- Create an ABR community scorecard for HIV services.
- Pilot the ABR index and community scorecard in Hubs, making necessary adjustments.
- Develop a training guide on the index and scorecard.
- Oversee peers in delivering 3-5 trainings to other providers and users.
- Conduct baseline data collection and analysis.
Project 2: Integrated National HIV Prevention Strategy for ACB People in Canada
- Adapt the Community Health Ambassador Model for Alliance Peer Coordinators specific to ACB sub- populations.
- Develop a Peer Toolkit to support the implementation of activities.
- Deliver workshops to diverse communities of ACB people and service providers.
- Develop and disseminate a framework for delivery and national advocacy strategy based on a status- neutral approach, barrier reduction, Anti-Black Racism, Intersectionality, Anti-Oppression, GIPA/MIPA, SGBA+.
- Create outreach/media materials and strategy, identify pop-up sites, and procure prevention technologies and external resources.
- Conduct 1Love1Stop intervention pop-ups.
- Conduct 1 community consultation to inform the design and development of Service Provider workshops.
General Responsibilities:
- Assess the needs of HIV-infected and affected clients, provide counseling, advice, and referrals to other services, agencies, or programs, and develop strategies for each individual.
- Conduct research on effective strategies and models for peer-based programming to inform program delivery.
- Provide system navigation support and identify appropriate referrals to complementary programming based on the determinants of health framework.
- Offer Prevention Case Management (PCM) and other case management support to Peers.
- Coordinate the development of intake and assessment tools to assess risk and factors related to risk.
- Provide ongoing system navigation support to Peers.
- Recruit, train, and place Peers to engage in peer activities at all levels of program delivery.
- Support the delivery of workshops on safer sex, disclosure, goal setting, and treatment adherence.
- Provide referrals as required to HIV/STI testing sites, other ASOs (AIDS service organizations), and services such as housing, mental health, and employment.
- Attend staff and team meetings, as well as relevant training sessions.
- Participate in staff and team meetings, develop monthly reports, and perform other assigned duties.
Qualifications:
- Bachelor’s degree in a related field (public health, social work, or similar)
- Experience in HIV/AIDS prevention and community engagement
- Knowledge of Anti-Black Racism and its impact on health outcomes
- Familiarity with peer-based programming and case management approaches
- Strong research and data analysis skills
- Excellent interpersonal and communication skills
- Ability to work collaboratively and independently.
- Proficiency in relevant computer applications

Greetings, Black Coalition for AIDS Prevention (“Black CAP”) Membership, community members and stakeholders.
Thank you to all the members who attended the Town Hall meeting on June 13, 2024. From that meeting members, staff, community and stakeholders voiced their concerns about governance and operations leadership of Black CAP. Your concerns were heard and since then the Board has been focused on addressing the crisis that the organization faces.
This notice is to inform you that Black CAP is calling a Special Member’s Meeting for
Thursday, July 18th, 2024, 6:30 pm (ET)
Registration will open at 5.30 pm

Location and meeting package details will follow by July 12th, 2024.
The purpose of the meeting is as follows:
To fill vacancies on the board of directors by electing members to serve on a Transitional Board of Directors of Black CAP for a period not to exceed one (1) year or until the annual general meeting of Black CAP in 2025.
This meeting and voting will be held in-person only. This is out of an abundance of caution due to previous concerns explained at last year’s Special Members Meeting about Proxy voter intimidation; hence we will not permit Proxy voting at this time.

The Board will endeavor to make publicly available the names, write-up and photos of director candidates received prior to Monday, July 15, 2024, 12pm, and reserves the right to decline posting any write-up or photo deemed inappropriate. All nominations received after will be treated as nominations from the floor.
In accordance with section 10(4) of Black CAP By-Laws, nominations will be accepted from the floor.
We must never forget what really matters: the lives of the communities, particularly our black communities living with and affected by HIV/AIDS, who desperately need the services and support provided by Black CAP.
